After seeing the status of Rafael's current puzzle and assuming Mellon's G and Ladyoflegolas's H are both correct, I guess it is up to me to fill in the other missing blanks and give the answer. It being:

The Great Year of Plenty found under T.A. 3020 (S.R. 1420) in Appendix B of RotK. I only looked it up after having solved it.

So now try:

_ _ _ _ ' _ / _ _ _ _ / _ _ _ _ _ / _ _ _ _ _ _ _ / _ _ _ _ _ !
(five words of 5, 4, 5, 6, and 5 letters length respectivly.)

any chance of putting which book each phrase is from?

Ladyoflegolas: The answer should be yes, but only after a puzzle has been correctly guessed, else they would become far too easy.
